All representative are allowed up to 10 incidents (10 points) per every 6 months. The 6 months will start on an individual’s first incident. After a rolling 6 month period, all occurrences will fall off at their individual 6-month expiration date.
Any call outs that are not scheduled 3 days in advance
If you are sick over multiple days with a doctor’s note will be 1 incident
Leaving early before your shift has ended
More than 15 minutes late to start your shift
Calling out 30+ minutes after the start of your shift and No Call No Shows are equivalent of 5 points
Please see below for disciplinary action for incidents:
By the 5 incident- verbal warning = Should be clearly documented in 1 on 1 spreadsheet
By the 7 incident- written warning = Rep must meet with supervisor & Carla. The warning is signed up representative and Carla
At the 9 incident - Final written warning = Rep must meet with supervisor & Carla. The warning should be signed by the representative and Carla. Chris should be looped in.
By the 10 incident- termination
